The BBC has released the first teaser for Casualty’s landmark 40th anniversary, confirming that a special double-bill will air on Saturday, 5 September 2026 on BBC One and BBC iPlayer. Titled “Life Begins at 40”, the trailer marks four decades since viewers first entered Holby City Hospital. Set to “Happy Birthday”, the footage quickly takes a darker turn as a huge fireball erupts over Wyvern while clinical lead Flynn Byron, played by Olly Rix, walks across Bristol’s Clifton Suspension Bridge.
Outside the emergency department, Dylan Keogh, Rash Masum, Rida Amaan and Cam Mickelthwaite watch the explosion in shock before emergency calls begin flooding the ambulance station. The teaser also shows Stevie Nash and Flynn fighting for survival in the back of an ambulance, armed police approaching the hospital and Dylan breaking down after receiving devastating personal news.
The final image reveals ambulance lights glowing beneath murky water, pointing towards the major rescue operation at the centre of the anniversary event.
The double-bill will follow a high-stakes incident that pushes Holby’s emergency department to its limits. As the fallout grows, the team faces impossible life-or-death decisions, with Dylan attempting to lead his colleagues through their darkest hour while dealing with tragedy of his own.
William Beck, Elinor Lawless and Olly Rix lead the episodes alongside Melanie Hill, Barney Walsh, Neet Mohan, Charles Venn, Sammy Dobson, Kirsty Mitchell, Di Botcher, Anna Chell, Sarah Seggari, Michael Stevenson, Milo Clarke, Naomi Wakszlak and Aron Julus.
Produced by BBC Studios Drama Productions, Casualty remains the world’s longest-running medical drama. The anniversary episodes will celebrate the characters and stories that have defined the series while beginning a new chapter ahead of further changes planned for its future.

Casualty was never going to mark 40 years quietly, so a major disaster feels fitting for the occasion.
